Athletes that have a physical on file from last year need to fill out this attached form. Those that don’t have a physical on file, will need to get one before the first practice.
Holy Family Catholic School has a no fee/no cut policy. Students are not charged to participate in our team sports. All students are encouraged to be part of our teams at the appropriate grade level.
ALL ATHLETES MUST HAVE A CURRENT PHYSICAL, DATED APRIL 15TH, 2022 OR LATER, TO PARTICIPATE IN PRACTICES OR CONDITIONING ALONG WITH A HOLY FAMILY PARTICIPATION FORM. PARTICIPATION FORMS CAN BE FOUND HERE. PHYSICAL FORM CAN BE DOWNLOADED HERE.
All athletes must also turn in a concussion awareness form found here. Parent information about concussions can be printed here.
Any parent interested in coaching should fill out this request, and turn into the Athletic Director.
PLEASE RETURN UNIFORMS WITHIN 3 DAYS AFTER THE END OF EACH SEASON – CLEAN AND IN A BAG WITH THE ATHLETES NAME CLEARLY MARKED.
